Where are we located?

We are located in the warehouses behind Neal's Restaurant. Follow the big wooden fence and the Redefined CrossFit signs. See picture below in the Contact Information section (Suite 502, second door on the right). 

Do we accept insurance?

Yes, we accept all major medical insurances, but we are IN-Network with BCBS. 

What forms of payment do we accept?

We accept cash, all major credit cards (CC), ApplePay, Venmo, checks (made out to Dawn Health & Wellness LLC), FSA and HSA CC. 

What do I wear?

We prefer loose fitting athletic clothes, but please wear whatever you are comfortable in. We may have you go outside and perform functional movements.

Cancellation/ No-Show policy

We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much-needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to seemingly "full" appointment book, If an appointment is not canceled at least 24 hours in advance you will be charged a twenty five dollar ($25) fee; this will not be covered by your Insurance company

Where is the new patient paperwork?

The new patient intake form will be sent via email for you fill out at your convenience prior to your appointment.
